Secondary Transition Services

The LEARN Student Support Services Department is committed to the success of students with special needs in schools and communities. Through vocational training and transition programs, LEARN strives to impact students, and their families with community participation and positive and sustainable learning. STS represents collaborations and partnerships with families, districts, state-based agencies, businesses, universities and legislators to create programs and open doors for transitioning 14-21 year old students with disabilities in Southeastern Connecticut. Transition Services include: Agency referrals, professional development, assessments, vocational evaluation, person centered planning, job coach, related services, parent education/consultation & workshops, transition resource library, social opportunities, and college and career readiness.
